Please print the form and fill out the information completely.  When finished, please mail it along with your registration fee to the address on the form. 

Crossroads/Radiant Discipleship Walk is an open community.

ANYONE may attend, regardless of his or her church affiliation.

Reservations are accepted first come, first served.
